To submit your request, please provide the information below, and print, sign, and return the form to the Department of Elections. You may also download a PDF version of the Cancellation Form, or contact the Department to receive this form by mail.
Please allow the Department 3-5 business days to process your request. To confirm the cancellation, use the Voter Portal or contact us.
To notify the Department of Elections that a different voter has moved, please submit a Third Party Notification Form. To report a voter’s death, please use the Notification of Deceased Voter Form.
