This online application is only for voters registered in the City and County of San Francisco.

To request a ballot by mail for the November 6, 2018, Consolidated General Election, complete and submit this form online no later than 11:59 p.m. on Tuesday, October 30, 2018.

To become a permanent vote-by-mail voter and receive a ballot by mail for every election, complete, sign and return a Vote-by-Mail Application to the Department.

According to state law, only the registered voter may apply for a vote-by-mail ballot. An application for a vote-by-mail ballot made by a person other than the registered voter is a felony punishable by imprisonment and/or by fine.
